A patient is released into a public ward. Anyone in the world takes a shift on her,
treats her for a few minutes, and hands over. The next person opens her where the last one
stopped — because the chart is rebuilt from the chain in your own browser, not handed to you by
us. Mistakes carry forward. The record says who, and nobody can rewrite it.
